How to Complete a Wellness Survey

Wellness surveys are a quick way to keep track of how Loved One is feeling. Completing the Wellness Survey will save a record that can be viewed later and allow your Care Circle to track your mood over time. The Wellness Survey can be completed by a Loved One directly, or from a Caregiver account.

Complete the Wellness Survey as a Loved One

This section of the article provides instructions for completing a Wellness Survey from a Loved One account. As a Loved One, you are the one who is the subject of the wellness survey. If you are unsure which account type you are have, click here for definitions of each type of account.

  • From the Home screen, tap the "How are you feeling today?" card
    This card appears once per day and will disappear once the Wellness Survey is complete or if you tap the "X" button to dismiss the card. If you accidentally dismissed the card, but still want to take the survey, follow the instructions in the next section titled "Complete the Wellness Survey as a Caregiver"
  • Select the appropriate mood from the list and tap "Next"
  • If "Good" or "Fine" were selected, a confirmation message will appear. Tap "Finish" to complete

  • If "Not Good" was selected, an additional list will appear. Select the reason for the not good feeling from the list
  • Tap "Next" to complete

Complete the Wellness Survey as a Caregiver

This section of the article provides instructions for completing a Wellness Survey from a Caregiver account. As a Caregiver you are completing the wellness survey on behalf of your Loved One. If you are unsure which account type you are have, click here for definitions of each type of account.

  • From the Home screen, tap the "Tracking" button at the bottom of your screen
  • Find the Wellness Survey tracking card and tap on it to create a manual entry. The Wellness Survey tracking card it titled [Loved One Name]'s Feeling

  • Tap the "+" button to begin a new entry

  • Select the appropriate mood from the list
  • If "Not Good" was selected, an additional list will appear. Select the reason for the not good feeling from the list
  • Enter a date and a time of day for the Wellness Survey
  • Enter in any additional notes about the survey (optional)
  • Tap "Save"
